1. The name of "the association" is Nurses' Professional Association of Queensland (NPAQ).
2. Payments will be treated as a subscription to and will be receipted by Red Union as my appointed agent ("the agent") who will where appropriate represent me as follows:
a) I authorise the agent to use part of my subscription fee as agreed with the association(s), to apply on my behalf for membership to the association(s) and I agree to abide by its Constitution, by-laws, policies and procedures. I understand my association membership application is subject to the terms as set out in this application.
b) I confirm that with my subscription to the agent, I am also joining a series of member-based professional association(s) that provides members various services.
c) I appoint a third-party nominated representative of my choice, mutually agreed to by the association(s) or the agent, or a representative of the association(s), the association itself, or the agent or its nominee, or jointly as above with the agent or its nominees to act as my bargaining agent or representative, and authorise them to represent me in any industrial matter or dispute. Such an appointment is to apply until withdrawn by me in writing.
d) I authorise the agent to act for me with respect to the relevant employee and/or professional association(s) nominated by me, including, if requested and authorised by me, to advise my resignation from any other professional association(s) and/or union(s) of which I may currently be a member.
e) I acknowledge and accept that by joining the association(s) which is "a company limited by guarantee", in the event that the association(s) is liquidated, I can be liable for no more than one (1) dollar.
f) I authorise Red Union Support Hub to establish an automatically renewing subscription payment at the interval selected by me during sign up.
g) I declare the details provided in this application are, to the best of my knowledge, true and accurate.
